How can I upload an Addendum as a provider?

If you need to add additional documentation once an assessment is complete, you can edit within a short window of time or add an Addendum.

  1. Search for the candidate/employee that requires an addendum in the closed work items.
  2. Next, Click the arrow button underneath the actions column and then click add addendum.
  3. Fill in the required fields, choose the file and click the upload button.
  4. Once the file has been uploaded, click the Upload Record button, the addendum is then added to the final report.

    Here is an example of how it will appear on a final fitslip report.
